A quick guide to adding medical conditions to Medical Tracker:
Here is a step by step guide:
- Login to your Medical Tracker account
- Click on the Medical Conditions tab
- Click on the Add Medical Conditions button
You will then see the form which will allow you to record a Medical Condition against a child within your school.
Student Name - The Student you are adding the condition for
Created By - The member of Staff recording the Medical Condition
Requires healthcare plan? - If yes, it will be recorded as a detailed care plan, including a review date. If no, then you will be given less detail to fill in.
- Press Add Medical Condition
Medical Condition - This is a suggestion box that will give you suggestions of Medical Conditions, but you can type anything you need into this box
Is this an ongoing condition? - A simple yes or no box
- Press Add Medication
Medication Name - This is a suggestion box that will give you suggestions of Medication, but you can type anything you need into this box.
Dosage - Free-text box to fill on as much as you need
Side Effects - Free type box that will allow you to fill out side effect issues
Date Dispensed - When the Medication was released by the pharmacy
Expiry Date - The date the Medication will expire
Self Administered - A simple Yes or No suggestion box
Special Precautions - Free type box that will allow you to fill out any precautions that need to be taken
Add Timing - allows you to record timings for Medication
- Press Next button
All of these boxes are free text boxes so that you can fill out as much information as you need to make sure the best care can be given to the student.
- Once you are finished, press Save
